Description:

The Advanced Practice Nurse (APN) provides advanced nursing care in the acute care setting, based on educational preparation and/or certification in the APN role to a specific patient population. Care goals include promotion of healing, prevention of illness and recidivism. The APN assumes the management of patients, functioning in both an independent and collaborative role with the physician, case manager, and other members of the health care team. As an expert practitioner, the APN performs selected invasive procedures, designs individualized treatment plans, evaluates outcomes, and modifies the treatment plan as needed. The APN facilitates the transition of patients between service units and provides/coordinates complex patient and family teaching. Recognizes and consistently applies knowledge of Loyola's 14 Magnet Forces.

Position Responsibilities: Ensures that care delivered to patients, meets/exceeds standards of care and is provided according to Loyola University Medical Centers mission, strategic plans and all internal/external regulations, policies and procedures. Functions independently and collaboratively with the physician to provide continuity of health care services for individuals, families or groups. Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on comprehensive history, physical exam, and psychosocial assessment. Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, identifies variations from the norm in diagnoses emergent problems and complications. Designs and evaluates individualized treatment plan. Demonstrates and maintains expertise in the performance of selected complex invasive procedures. Coordinates care for optimal continuity of care across the health care continuum. Facilitates patient/family participation in health care by identifying learning needs and providing specialized education to enhance decision making. Exhibits professionalism by performing research, education, and leadership in clinical practice.

Position Requirements:

Minimum Education: Required: Masters Degree

Minimum Experience: Required: 3-5 years of previous job-related experience

Preferred: 6-10 years of previous job-related experience

Licensure/Certifications: Required: CPR (Cardiopulmonary Resuscitation), Current Registered Nurse License State of Illinois, Nurse Practitioner License State of Illinois
