GENERAL SUMMARY AND PURPOSE:

Provide evidence based brief individual, group, family and educational services including assessment and counseling for the Saint Alphonsus Medical Group's primary care clinics through Population Health in Nampa, Idaho and Baker City.

Participate as a member of a multidisciplinary treatment team to help assure the most appropriate treatment of patients. Document clinical and consultative activities in patient charts. Work to develop and promote the programs and objectives of the department.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

  • Independently licensed (LCSW) to practice in the State of Idaho
  • Minimum of one year of post-license experience in related mental health setting preferred
  • Experienced in crisis intervention and brief therapy techniques

ESSENTIAL FUNCTIONS:

  • Meet Health System's Core Values and Caring Standards including interpersonal communication and professional conduct expectations

  • Demonstrate knowledge and skills to competently care for all assigned age groups (Child, Adolescent, Adult and Geriatric as applicable)

  • Conduct initial evaluation of patients and communicate assessment information to physicians, APPs and nursing staff

  • Ability to clearly document at each visit objectives, measurable goals and progress toward goals and identify the expectations of brief counseling with patients

  • Ability to recognize when patient has achieved maximum benefit from therapy and professionally terminate care when patient is not progressing toward goals despite any needed treatment plan modifications or when patient has reached their goals

  • Counsel patients and families regarding recommended levels of care, community resources and financial arrangements
