Position Summary:

LifeMoves is seeking a highly motivated Bilingual (Spanish) Case Manager to serve our clients. Under the supervision of the Program Director, Case Managers act as strong supports and advocates for clients, with the ultimate goal of helping them attain stable housing and self-sufficiency. Their responsibilities include developing and implementing individual case plans for a caseload of clients, as well as assisting the Program Director to maintain the facilities and develop program services.

Position Description:

  • Outreach to community partners, the local community, and homeless families to promote program services
  • Develops, implements and revises individual case plans and consistently monitors progress toward client goals including housing and employment
  • Provide on-going supportive and/or case management functions in accordance with the case/treatment plan
  • Conduct intakes for program participants and screen all members of the household for safety
  • Communicate appropriately as needed with probation officers, social workers and family members
  • Participates actively as a team member in supporting clients and the facility (parking location) as needed
  • Stay current and knowledgeable about external community resources, referrals and service linkages
  • Provides crisis management for clients; makes linkages for interventions as appropriate
  • Facilitate at least one clinical or life skills group per week from program participants
  • Maintains client and program records in accordance with applicable standards and regulations, grant requirements, etc.
  • Maintains a high level of ethical conduct regarding confidentiality, dual-relationships, and professional stature
  • Participates in continuing education activities, remaining knowledgeable in area(s) of expertise
  • Willing and able to follow safety precautions regarding covid19, and work with team, clients, and community partners in person during covid19 pandemic.
  • Attends weekly case consultation meetings and monthly training; meet regularly with supervisor to exchange pertinent information and receive supervision
  • Other duties as assigned by supervisor
Position Qualifications

  • Bachelor's degree in social work or related field preferred
  • Minimum two year experience in serving homeless families and/or individuals in crisis.
  • Must possess a thorough understanding of the issues faces by chronically homeless individuals and families
  • Personal or professional experience working with individuals with addiction or mental health
  • Experience with outreach services for homeless individuals a plus
  • An understanding of ideas and practices that promote diversity, equity and inclusion; and a commitment to ongoing professional development in this area.
  • Must be computer literate and proficient in Microsoft Office
  • Bilingual Spanish-speaking required
  • Automobile and valid driver's license preferred
