Case Manager (Sr Nutrition) Riverside County

Salary: $18.00 per hour with benefits (Non-Exempt)

Schedule: Monday - Thursday 8 AM- 5 PM; Friday: 8 AM - 2 PM

Essential Job Duties:

Specific duties and responsibilities may include, but are not limited to the following:

  • Develop a full case management program for the senior nutrition program that both meets the basic nutritional assessment requirements of DAAS (San Bernardino County) contracts
  • Provides resources/referrals to assist senior clients to meet their needs
  • Assignment of cases and supervision of assigned cases, oversight that required paperwork is completed within required timeframes and guidelines.
  • Supervise and train volunteers and interns as assigned.
  • Conduct minimum of 30% in direct time with clients to provide assessment services.
  • Ensure that between own workload and volunteer case workers all required nutritional assessments are completed in a timely manner.
  • Work with support staff to ensure that data input of all nutritional assessments is completed timely.
  • Coordinate ongoing case worker training in collaboration with program managers and administrator.
  • Assist agency staff in program development and grant writing as assigned for additional financial support for case management services for seniors.


  • Report to: Program Administrator

    Minimum Qualifications:

    MS Office Application Experience (Word, Excel, Access)

    Experience with volunteer coordination

    Valid CA driver's license and Proof of Auto Insurance

    Background, Fingerprint Clearance and Drug Test Clearance

    The ability to read and write English and to communicate at the level required for successful job performance.

    Licenses & Certifications:

    Bachelor's Degree in Social Work or a related field OR an AA degreed paraprofessional case worker with significant experience in case management with seniors may be considered

    Valid California Driver's License, clean driving record, and proof of automobile insurance
