ABOUT THE JOB
The Personal Recovery Oriented Services (PROS) program aims to help each participant achieve their chosen life goal related to housing, work, education and/or building their socialization skills. The services are provided using a recover-orientated, person-centered strengths-based approach with the goal of empowering individuals to improve their quality of life, achieve life goals as they take a more active role in their community.

The Case Worker is responsible for supplying necessary clinical intervention within the realm of the program. Provide cultural competent and sensitive services. As part of a therapeutic team within the Agency, work closely with the divisions' medical, psychiatric and psychology staff, to ensure the appropriate treatment interventions for the client. Specific responsibilities include:

  • Coordinating all services for consumers, including day programs, medical care, transportation, summer camps, summer programs, recreational activities, and community inclusion (Person Centered Planning).
  • Liaising between Department and consumer's family members, and relevant community members.
  • Leading weekly team meetings. Keeping minutes of team meetings and ensure follow-up as necessary from meetings.
  • Conducting meetings to ensure proper treatment, progress in program and coordination of goals.
  • Attending annual case conference at day program or school.
  • Training Direct Care Workers, as necessary.
  • Participating in audit preparation.
  • Working with intake coordinator on intake process for new consumers.
  • Working with Direct Care Workers to enhance communication and cooperation between them and consumer families.
  • Completing paperwork in a timely manner.
  • Other duties as assigned.

TO QUALIFY
The Jewish Board is currently looking for talented Case Workers of all cultures, religions, races, and gender expressions with the following qualifications:

  • Master's degree in Social Work, (MSW), or Master's degree in mental health field with a minimum of one year of relevant experience with a similar population.
