Description:
The Community Homeless Outreach Program (CHOP) team provides assertive outreach, case management, and health screenings and education to clients experiencing homelessness in places where clients congregate. The mental health specialist, senior will provide services alongside the CHOP team in the field as well as alongside the PATH team in the clinic. These services will be heavily focused on housing case management and connection to behavioral health services through Mercy Care clinics.
This is a specialized service position providing assertive community outreach and mental health and substance abuse (MH/SA) support in the direct rehabilitation of persons with such diagnoses. Services may include MH/SA screening, intake, orientation, recovery planning, case management, client education, referral, and reporting and record-keeping. This individual will work alongside a nurse and resource specialists on the Community Homeless Outreach Program (CHOP) to provide services to clients in their own environments. He/she will also work alongside other mental health specialists and a peer support specialist in the Projects for Assistance in Transition from Homelessness (PATH) team to provide services to clients in the clinic setting and on outreach. Participates in community meetings on a monthly basis and reports information to team. Manages an average caseload of 18-22 clients. Also participates in health fairs or community events on weekends as requested 3-4 times a year.

*Provides patient care and/ or support activities appropriate to ages served; primarily adults ages 18-65 or geriatric patients ages over 65. May also include care for infant age 0-1 year, child ages 1-12 years or adolescent ages 13-17 years.
