General Summary

Working within a multidisciplinary team of psychiatrists, psychologists, resource specialists and patient service coordinators, the Psychiatry Triage Clinical Care Social Worker is responsible for providing clinical support to the Outpatient Psychiatry Department. The social worker will work with the Psychiatry Access Department and the Outpatient Psychiatry Message Center which collectively serve as patients' starting point for engaging in services and the primary point of contact with the department and its providers. The Triage Clinical Social Worker will work alongside the department's PTIS team including Patient Service Coordinators and Administrative Coordinator as well as the department Resource Specialist, outpatient psychiatry department social workers and UCC Social Worker in order to provide clinical assessment for triage purposes and to connect patients with community-based resources. This position requires a high degree of flexibility and willingness to participate in program planning as this new role within the department evolves.

Principle Duties and Responsibilities

Clinical Access
  • Provide clinical support to BA level intake and triage team, completing complex intakes over the phone with patients when indicated, and assisting the staff in determining which clinical services are the most appropriate fit for a patient.
  • Refer patients to specialty clinics within MGH when diagnostically appropriate
  • Manage patient calls that require an on-the-spot clinical assessment of the severity of a patient's symptoms and/or potential threat of harm to self or others, and implement appropriate interventions
  • Assist patients who do not qualify for care within MGH in navigating community resources accessible to them based on insurance, geographical location, and access to transportation
  • Participate in the patient transfer process when a provider leaves the department, working with the departing provider and conducting chart reviews when indicated to reassign patients to a provider who is an appropriate fit
  • Work with PTIS program and administrative managers are larger PTIS projects regarding patient access including running no-show reports, overseeing the referral system and reviewing referrals for urgency
  • Advocate for patients to receive access to care in a timely manner
  • Document all telephonic encounters in Epic, schedule patients appropriately for appointments in Epic
  • As this is a new position, the Clinical Care Social Worker will participate in process improvement projects with Administrative Team helping to guide the development of the role so that it functions optimally and efficaciously within the behavioral health system.
    • This may include but is not limited to: data collection and analysis, taking initiative to suggest and test new processes and pathways for accessing and delivering care, and collaborating with a multidisciplinary team to improve the overall patient experience.

Clinical Resource Coordination
  • Work with other department social workers to address clinical issues and workflow questions
  • Provide coverage as needed for other Clinical Care Coordinators and UCC Social Worker. This may involve performing referrals to higher levels of care, scheduling of appointments to departments within MGH or in community, or assisting with urgent clinical needs and assessment
  • Assist with triage of complex patient referrals to locate appropriate care at MGH or in community
  • Educate outpatient providers as to what additional services patients may be able to access, both internally and in the community. This may include delivering presentations to the various clinical groups.
  • Provide psychoeducation to patients/families regarding illness, highlighting protective factors and working together define adaptive coping strategies
  • Perform other duties to support clinical care of patients as directed
  • Attend and participate in all required interdisciplinary meetings and staff meetings

Qualifications
QUALIFICATIONS

-Bachelor's degree in social work, psychology, counseling or a related field

-Master's Degree in Social Work Required, LCSW Licensure Required

-Minimum of 2 years work experience in the mental health care field required
