Summary:
The Community Patient Navigator will work to increase breast and prostate cancer screening rates in Federally Qualified Health Centers (FQHCs) and community health centers in the WNY region through targeted and culturally tailored education and navigation, primarily serving the Erie and Niagara County. The PN will also offer support and assistance through the screening and any potential diagnostics and treatment of patients as needed; navigate and tracks patients through health care systems, including primary care offices, hospitals and outpatient clinics, insurance and payment systems, patient-support organizations, cancer services programs and other components of the health care system. The PN will also help identify barriers preventing patients from receiving timely and high quality healthcare treatment and work to find solutions; ensures that continuous quality care is received by all patients; assists with connecting patients to community and clinical referrals for assessment and treatment; assists patients in accessing needed resources and services to move through the spectrum of cancer care; works with internal services at the FQHCs and in conjunction with primary care providers (PCPs) to coordinate internal and external services, outreach programs, local and regional health centers and transportation services to facilitate care. They will serve as a resource for cancer screening services and management of co-occurring conditions along with PCPs and clinic staff. Uses Electronic Medical Record (EMR) systems to identify and flag patients due for cancer screenings for targeted education and navigation. Work closely with Senior Health Referral Specialist, Senior Community Outreach and Engagement Manager, Principal Investigator(s), and key clinical stakeholders to define and execute a workflow meeting the needs of the community and practice.

Starting salary for this position is $45,960 annually which includes a comprehensive benefits package.

Qualifications:
Required Education and Experience

License Requirement
Valid New York State Driver's License.
Education and Experience
1. Bachelor's degree in a health-related field and the equivalent of one (1) year of full-time experience in community, patient or customer oriented support; or
2. Associate's degree in a health-related field and the equivalent of two (2) years of full-time experience in community, patient or customer oriented support; or
3. High School Diploma or High School Equivalency Diploma and the equivalent of four (4) years of full-time experience in community, patient or customer-oriented support.

NOTE:
Required degrees must have been granted by an accredited college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.

Preferred Qualifications:

The preferred candidate will have their associates or bachelors degree with a minimum of three years full time experience working in patient navigation, experience working with community health centers and/or Indian Health Service clinics, previous experience working in EMR systems and experience working with Community Coalitions, patient education and navigation.
