Job Description

Lyndale Manor-VOA is looking for a Director/RN Case Manager join the team. The role of the DOHS is to assess, implement review resident service plans, Coordinate care with case managers and social workers, and oversee 2 nurses.

*** This is a smaller Assisted Living/Independent site with up to 45 units of residents. Experienced ADON, RN /Nurse Manager interested in a "step up" or an established DOHS, encouraged to apply!

About Lyndale: Lyndale Manor public housing building located in North Minneapolis. Supported by Volunteers Of America National Services with assisted living services onsite. This program allows residents of the neighborhood to remain living independently and safely where they are connected. CORE RESPONSIBILITIES:

• Accepts responsibility for the facility
• Instructs staff on various, state and facility's regulations, policies and procedures; monitors compliance.
• Participates in the preparation of the operating and capital budget .
• Monitors department expenditures to budget.
• Oversees and directs day-to-day functions of the staff
• Review resident service plans
• Asses and increase admission to the program
• Staff development
• Resolve resident and family concerns

Job Requirements

QUALIFICATIONS:

• Registered Nurse with minimum of two (2) years experience in nursing supervisory or nurse management in an assisted living setting or equivalent.

• Demonstrates knowledge of assisted living, gerontology, and Infection Prevention and Infection Control.

• Knowledge of Minnesota Assisted Living regulations
