LPN - Schenectady/Albany Counties
If you are looking for an LPN position helping the Hospice Home Care Team caring for patients and their families in the community this could be the position for you! The Community Hospice has a unique opportunity for an LPN to work Full-Time (Monday-Friday).

What you will do:
- Coordinate closely with ourRN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families
- Work closely as part of a interdisciplinary team
What you will need:
- Valid NYS LPN License
- Minimum 1 year LPN experience
- Prior hospice/ home care experience preferred, but not required
- Strong computer skills
- Organization and communication skills and ability to work as part of a team
