Schedule: Night shift, 8p-4:30a Sat/Sun

Location: NGMC Gainesville Telemetry

Department Overview:

The Central Monitoring Unit (CMU) provides continuous monitoring of a patient's cardiac and respiratory rhythm, and/oxygen saturation using cardiac monitors and pulse oximetry.

Located on the Gainesville campus, CMU has 11 central monitoring stations, with each station having the capacity to monitor 48 patients. CMU monitors Gainesville, Lumpkin, Barrow and Braselton critical care patients and acute care patients with provider orders.

CMU is responsible for notifying the patient's primary shift RN with changes in, or alarms related to: oxygen saturation, respirations, cardiac rate and rhythms, ST analysis, poor signal, leads off or low battery signals. A dedicated emergency phone line is maintained on each nursing unit for CMU communication of life threatening or potentially life-threatening dysrhythmias.

CMU records and interprets findings in the monitoring system. These records interface into the EMR every 12 hours.

Job Summary:

The Monitor Technician is responsible for on-going surveillance and advanced ECG interpretation for critical care and telemetry patients. The Monitor Technician observes cardiac monitors, identifies, calculates, interprets, documents, and communicates information regarding cardiac rhythms to nursing staff and/or clinicians. Identifies, interprets, documents, and responds to potentially life-threatening rhythm changes. Initiates emergency response by calling on an emergency phone line or initiates a code blue or rapid response if necessary, per policy. This position provides cardiac and pulse oximetry surveillance via telemetry in all age ranges exclusive of neonates.

Minimum Qualifications:

  • High School Diploma or GED
  • Must have Basic Life Support Certification through the American Heart Association
  • Previous clinical experience preferred but not required
  • Previous successful completion of EKG Technician course/certification preferred but not required

Essential Tasks and Responsibilities:
  • Clinical Competency
  • Participates in mock codes and other emergency preparedness drills or events.
  • Completes electronic health record (EHR) training.
  • Successfully (100%) passes a yearly rhythm strip/monitor tech test.
  • Demonstrates knowledge of dysrhythmias and conduction abnormalities with recognition of their potential risk as evidenced by appropriate documentation and timely notification of the shift primary nurse.
  • Obtains baseline ECG tracing on admission or initiation of telemetry monitoring.
  • Monitors lead II and V1 or lead as directed by the shift primary nurse. Troubleshoots ECG monitoring problems. Monitors heart rate, rhythm, and electrical conduction for abnormalities. Admits the patient, initiates monitoring including pulse oximetry, retrieves data, activates standby mode, and discharges the patient per order.
  • Assists the provider and shift primary nurse in obtaining monitored documentation of urgent events.
  • Follows all provider orders at the direction of the shift primary nurse.
  • Precepts monitor techs as needed.
  • Recognizes and reports changes in the patient's condition in a timely, accurate manner. Prioritizes patient problems appropriately as evidenced by immediate attention to those with actual or potential urgency.
  • In collaboration with the shift primary nurse, the monitor tech will adjust alarms to meet patient needs.
  • Documentation of Patient Care
  • Demonstrates knowledge of documentation standards as evidenced by the appropriate use of the EHR including completeness of entries and use of approved abbreviations. Demonstrates a standard practice of concurrent documentation (except in extreme circumstances) to ensure accuracy and thoroughness.
  • Maintains patient clinical information for telemetry monitoring. Interprets ECG strips every four hours and as needed. Ensures the data on each patient is pushed electronically into the EHR.
  • Documents ST segment analysis measurements in one or more ECG leads and notifies the shift primary nurse. Indicates changes in lead placement as appropriate.
  • Confirms when telemetry is initiated and discontinued.
  • Documents time and content of notice given to the nurse of dysrhythmias, conduction disturbances, and monitoring strip entries. Notifies nursing staff of transmission issues.
  • Interdisciplinary Continuity of Care
  • Communicates and collaborates with other healthcare team members to implement patient care practices in response to identified needs.
  • Communicates data effectively through organized, accurate, thorough, and concise reports.
  • Anticipates needs and makes proactive plans to prevent errors or delays.
  • Uses hand-off tools appropriately including shift change report and hand-off for breaks.
  • Customer Satisfaction
  • Controls noise levels in the unit.
  • Answers phones in a timely and courteous manner. Transfers calls and relays messages as needed. Maintains confidentiality of patient information when giving phone condition reports.
  • Management of the Environment of Care
  • Assists in maintaining sufficient supplies, operable equipment, and attractive appearance of the unit. Notifies appropriate persons of needed repairs or supplies and follows through as indicated.
  • Operates printer, copier, and fax machine. Maintains readiness for use by changing cartridge, replenishing paper, and reporting problems through Service Now.
  • Other duties as assigned.
  • Accepts an assignment on another station when requested by leadership for support of the overall patient care program.
  • Participates in performance improvement (PI) activities as assigned. Provides PI monitoring, results, and recommendations. Complies with patient care/documentation practices targeted for improvement.
