GENERAL SUMMARY

The Addiction Consult Team (ACT) Nurse Practitioner is a part-time position with the Massachusetts General Hospital Substance Use Disorders (SUDs) Initiative, hospital-wide program to improve access to evidence-based substance use disorder care. This position is primarily based on the Addiction Consult Team, an interdisciplinary team that provides consultation to patients with substance use disorder who are admitted to the hospital. This nursing support will provide clinical oversight of the consult triage process and discharge follow-up outreach to patients seen by ACT as well as other responsibilities indicated below. This role will also provide support to a new clinical trial designed to initiate extended-release buprenorphine in the inpatient setting, for which ACT is a site. NP/CNS applicants accepted.

PRINCIPAL DUTIES AND RESPONSIBILITIES

  • Provide competent, compassionate, evidence-based nursing care utilizing the nursing process and interdisciplinary teamwork, and collaboration.
    • Provide post-discharge follow-up outreach to patients cared for by ACT to ensure that the patient successfully transitions to the next step in care. Work to address any barriers that impact the patient's access to care after discharge.
    • Facilitate patient group(s) for patients in the hospital and cared for by ACT
    • In certain situations, serve as the consulting clinician for ACT patients, providing diagnosis, treatment, education, and management recommendations to the ordering provider
  • Provide daily clinical oversight to the distribution of consults
    • Together with the ACT Coordinator, review the new consults each morning as well as the current patient list each staff member is carrying to determine appropriate distribution of new consults or re-distribution of existing consults if there are staffing challenges
    • Identify consults that may not be appropriate or for which more information is needed and contact the order provider to gather that information
    • If some consults need to be deferred to the next day, identify which consults are most urgent and which can safely be deferred. Communicate with the ordering provider if needed.
  • Provide support to the clinical trial designed to initiate extended-release buprenorphine in the inpatient setting
    • Coordinate with the research nurse team who will administer the rejections
    • Monitor the patient after injection
  • Provide leadership in the practice to ensure safe, efficient and cost-effective care.
    • Identify opportunities and solutions, to ensure a safe, efficient and cost-effective practice environment
    • Demonstrate compliance with regulatory requirements
    • Participate in quality and safety initiatives
    • Generate innovative ideas to improve care delivery and work flow in the practice environment
    • Participate in departmental and organizational committees and initiatives
  • Professional Development
    • Maintain and updates clinical knowledge and skills based on current nursing practice.
    • Adhere to departmental requirements for required training.
    • Identify specific learning needs and goals and collaborates with nursing leadership in developing a plan to meet them.
  • Any and all other duties as indicated within scope of practice


  • Qualifications
    SKILLS/ABILITIES/COMPETENCIES REQUIRED

    BLS Certification

    Certified Addictions Registered Nurse (CARN) certification required or willingness to achieve CARN certification in first 9 months of employment

    Proficiency in toxicology screening and addiction pharmacotherapy treatment options, including long acting injectable medications (or willingness to learn)

    Understanding of the philosophy of harm reduction

    Strong interpersonal skills.

    Strong patient communication and patient education skills.

    Excellent written and verbal communication skills

    EDUCATION

    Masters in Nursing

    EXPERIENCE

    2-4 years clinical experience preferred with experience treating patients with substance use disorders

    LICENSES, CERTIFICATIONS, AND/OR REGISTRATIONS

    NP/CNS applicants accepted

    Current licensure a Nurse Practitioner or Clinical Nurse Specialist in the Commonwealth of Massachusetts, and Mass Controlled Substances Licensing and X Licensing for Buprenorphine
