Responsibilities

Position Overview:

The Orthopedic Patient Care Navigator is responsible for the overall coordination and implementation of CMHS's Total Joint Replacement Program. The Navigator will be guiding the orthopedic patient and their family through the program from pre-operative education through the continuum of care. The Navigator works in collaboration with Orthopedic surgeons and Hospital departments, including Surgery , Physical Therapy, Case Management, Nursing, Quality and Marketing to develop, enhance and promote the Total Joint Program.

Qualifications

Required:

• Bachelors Degree or required within 2 years of hire
• Current California RN License
• Five (5) years clinical experience in a healthcare setting

Preferred:

• Masters degree in healthcare related field
• Orthopedic experience
• Case Management or Clinic Liaison experience
