Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 327-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus as well as at 10 Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The Outpatient Clinical Dietitian Specialist proactively contributes to Eskenazi Health's mission to Advocate, Care, Teach, and Serve with special emphasis on the vulnerable population of Marion County. The Outpatient Clinical Dietitian Specialist serves as the nutrition expert and contributing member of the healthcare team providing patient focused medical nutrition therapy, education, and care management in complex patients. Provides nutrition care in a variety of settings, including but not limited to the outpatient health centers, public outreach clinics, mental health centers, and community-based (e.g. schools, churches) settings.

Essential Functions and Responsibilities

• Provides patient-centered medical nutrition therapy using the Nutrition Care Process (NCP): Evaluates the nutritional status of the patient, develops care plans following the established standards of care, provides nutrition counseling to patients and their families that is age-specific, cultural, religious, and ethnic sensitive for health and disease promotion/prevention and wellness; monitors in accordance with the patient's treatment goals.
• Coordinates care between visits with the patient and health care team in order to update the plan of care; refers patients for community or home health services as appropriate and documents these in the patient's electronic health record (EHR).
• Maintains accuracy in documentation, including NCP format, charge entry and reimbursement standards, along with scheduling and referral management, while meeting established productivity metrics and assigned deadlines.
• Utilizes prescriptive authority within the regulations of the institution and legislative bodies to order diets, supplements, enteral nutrition and approved supplies.
• Educates consumers and professional audiences by incorporating a variety of learning strategies, behavioral and readiness change techniques, learning principles, and culturally and literacy sensitive educational media tailored for the client.
• Facilitates multidisciplinary team, clinic and dietitian team meetings, and case conferences.
• Maintains relationships with external customers to utilize latest technologies for optimal patient care and educates dietitians and nursing staff on clinical practice integration.
• Delivers care in high risk specialty clinics related to certification held and/or delivers content in nutrition education program which includes: internal promotion of programs; developing program education materials and interactive teaching tools; oversight on screening tools and recruitment; collecting data through reports/flowsheets, interpreting data and communicating results, and maintaining retention of groups
• Serves as a mentor for interns, new staff, or dietitians applying for advanced practice certification.
• Communicates and/or presents findings internally and/or externally of Eskenazi Health. Promotes Eskenazi Health and nutrition programs to expand the role in the community.
• Plans and oversees delivery of department nutrition programs: Selects qualified instructors and provides continuing education as needed; develops, coordinates and implements schedules for staff and program locations; supports program instructors with recruitment, enrollment and class implementation; maintains program inventory and effectively communicates program needs; prepares annual reports to be submitted to leadership while actively monitoring and evaluating program effectiveness through active outcomes measurement.

Job Requirements

• Registered Dietitian who maintains active registration, including continuing education, by the Commission on Dietetic Registration of the Academy of Nutrition and Dietetics; Certified Dietitian in the state of Indiana.
• Bilingual in English/Spanish preferable
• Nutrition specific certification applicable to the role (such as Certified Diabetes Educator, International Board Certified Lactation Consultant, Certified Nutrition Support Clinician, Certificate in Clinical Pediatric Nutrition, Lifestyle Medicine Certification) or functioning in team coordinator role.
• Minimum 5 years' experience with Master's Degree strongly preferred, or 10 years' experience in specialty areas required

Knowledge, Skills & Abilities

• Knowledge of Occupational Safety & Health administration (OSHA), The Joint Commission standards (JCAHO), Centers for Medicare and Medicaid Services (CMS), Federally Qualified Health Centers (FQHC), and Indiana State Department of Health regulations as it relates to nutrition care, patient education and safety
• General knowledge of nutrient analysis, word processing, spreadsheet software and other computer programs as required for patient documentation, patient health information, and departmental related reports and projects
• Excellent written and verbal communication skills in dealing with diverse patient populations and health care team member. Excellent understanding of technology and its integration into clinical care.
