Description:

Trinity Health Of New England is looking for an experienced Patient Placement Care Manager - Registered Nurse to join our team of mission-driven healthcare innovators at Saint Francis Hospital. This person is responsible for the coordinating the placement of all admissions (elective, direct and emergent) as well as transfers in accordance with clinical standards and guidelines

The ideal candidate will coordinate, prioritize, and optimize patient flow throughout the hospital, as well as coordinate, and facilitate patient entry from other health care facilities/organizations. The Patient Placement Case Manager works to ensure the timely and seamless clinical transition of patients through medically appropriate levels of care.

Work Hours/Shift:

You Will Be Responsible For:
  • Exceptional Patient Care: Contributes to overall quality of nursing care through performance improvement techniques that impact patient care positively.
  • Excellent Communicator:. Uses skilled interventions and evidence based practice knowledge to collaborate (patient, physician and multidisciplinary treatment team) and achieve quality outcomes.
  • Strong Educator: Educate patients and family members so that they have a transparent understanding of the care being provided.
  • Technical Familiarity: Understanding in computer-based applications (E-MAR, clinical documentation), email, and standard applications such as Microsoft Office.
  • Active Teamwork: Demonstrates problem solving, conflict resolution, and positive communication through teamwork.

Requirements:
  • Education: Graduate of an accredited School of Nursing.
  • Licensure: Current Licensure as a registered nurse (RN) in Connecticut
  • Certification: BLS certification required
  • Experience: 5 or more years of Clinical Nursing experience with previous experience in Critical Care or the Emergency Room desirable
  • Ability to pass drug screenings and background checks
  • Ability to work flexible hours and modify workload as needed.

Preferred Skills:
  • Education: Bachelors of Science in Nursing (BSN) degree preferred
  • Experience: Previous case management experience
