SUMMARY
Under the direction of a Registered Pharmacist, assists Pharmacists in preparing and distributing medications, maintaining the drug
inventory, and maintaining patient records. Provides courteous, cooperative, and timely service to patients, visitors and staff.

QUALIFICATIONS
Education
Minimum High school education or equivalent.
Preferred Completion of a formal Pharmacy Technician training program; or two years of college with emphasis in math, science, or medical field applications.
Credentials/Licensure
Minimum Licensed as a Pharmacy Technician in the State of Michigan.
Preferred Active status as a Certified Pharmacy Technician (CPhT) by the Pharmacy Technician Certification Board (PTCB).
Related Experience
Minimum Two years of pharmacy work experience. At least one year as a Pharmacy Technician III with consistent experience making simple and complex IV Admixtures (for inpatient pharmacy work locations).
For Medication Historian positions: 6-12 months successful performance in medication historian role after training (i.e., at Tech III classification). During this time, the medication historian should have progressed to and be practicing at a role-model level of performance considering both productivity and quality of work.
Other Knowledge, Skills and Abilities
Minimum Good written and verbal communication skills. Good math skills (multiplication, division, addition, subtraction) in calculations involving fractions, decimals, percentages and volumes.
Computer Competency
Familiarity with standard desktop and windows-based computer system, including email, e-learning, intranet and computer navigation. Ability to use other software required to perform essential functions.
Physical/Mental
Minimum Repetitive use of hands and fingers (e.g., preparing IV admixtures, use of a computer keyboard). May require lifting and carrying loads up to 40 lbs., including boxes, equipment, unit dose-cassettes,
and IV solutions and stooping or kneeling (e.g., to pick up items from the floor, to remove and replace items on lower shelves, and to file documents in lower file drawers). Sitting, walking, or standing for long
periods of time (4-8 hours) often. Must be able to physically operate the equipment for the job. WORKING CONDITIONS
Potential for exposure to infectious patients and materials. Potential for exposure to hazardous and toxic substances (including
chemotherapy, cytotoxic drugs, and cleaning solutions). Potential for sticks or cuts by needles and other sharp items. Potential for musculoskeletal injuries if proper lifting and carrying techniques are not used.

• Prepares, packages, and distributes medication orders per physician request, pharmacist request, or prearranged work assignment according to established policies, procedures and protocols.
• Maintains adequate stock of medications and supplies according to established policies and procedures.
• Contributes to effective and quality operation of the department.
• Maintains logs, records, and other required files and documentation; answers telephone and pneumatic tube; and handles cash transactions.
• Prepares IV admixtures and other sterile products.
• Processes medication charges and credits according to established policy.
• Chemotherapy/Hazardous Compounding Qualification
o Performs basic pharmacy computer order entry.
o Prepares chemotherapy and hazardous medication admixtures.
o Prepares and maintains equipment used for admixture of hazardous sterile admixtures.
• Controlled Substance Technician
o Controlled Substance issue and delivery from Pyxis CII Safe to Pyxis machines
o Controlled Substance diversion detection assistance as directed by Pharmacy Manager.
o Controlled Substance removal from returns bins and replenishment into CII Safe or Pyxis machine
o Monitors Pyxis medstations for unresolved discrepancies. Reports to nurse manager unresolved discrepancies in time period set by Pharmacy manager.
o Manages standard stock in Pyxis machine and removal of medication not used in time parameter set by Pharmacy Manager
• Buyer/340B Buyer Qualification
o Proficient in all Pharmacy Wholesaler system functions and processes for ordering, backorder processing, receiving, and returning products.
o Proficient in all Macrohelix processes for splitting orders, reviewing accumulators, and core functions for daily system maintenance.
o Proficient in placement of orders for controlled substances using CSOS and manual ordering processes
• Trains and participates in competency validation of Technician II, III, IV and personnel training for Technician II, III, and IV roles.
• Participate in the training, education, precepting, and evaluation of pharmacy students and/or residents as assigned.
• Performs other related duties as assigned. For Medication Historian Role:
• Solicits, collects, and records mediation history of patients in serviced areas into eHR.
• Develops Best Possible Medication List using both internal, external, electronic, print, and other resources (e.g., pharmacies, eHR).
• Seeks assistance in clarifying medication history information from other team members (e.g., ED pharmacists) as needed
• Actively participates in on going quality improvement and/or learning from defects discussions/meeting in support of high-level, accurate delivery of patient medication histories/lists. SYSTEMS AND INFORMATION

Accountability Act (HIPAA) and Saint Mary's Confidentiality of Information Standards, as well as other regulatory entities, individuals employed in this position will be granted systems and information access as appropriate for this position.
