Description:
The nurses on Acute Rehab treat patients who will benefit from an intensive, multidisciplinary rehabilitation program. Our program is fully accredited by the Commission on Accreditation of Rehabilitation Facilities (CARF), with an additional CARF accreditation for our stroke specialty program. Our nurses work closely with both our physical therapy partners as well as our dedicated physical, medicine and rehabilitation physicians.
Utilizes current nursing practices to provide patient care while focusing on the needs of the patient and exemplifying the service excellence goals of care, concern, respect and cooperation. As a contributing member of a health care team, develops and provides a comprehensive plan of care for assigned patients.

Minimum Education: Bachelors Degree

Licensure/Certifications:
Required: CPR (Cardiopulmonary Resuscitation),Current Registered Nurse License State of Illinois
Preferred: PALS (Pediatric Advanced Life Support),Neonatal Resuscitation Program (NRP) Recognition
Responsibilities:5% Quality Improvement - Utilizes time and resources effectively to promote quality and cost outcomes (Magnet Force #'s 1,7).,5% Professional Development Activities - Demonstrates accountability for professional growth and job performance by seeking opportunities to enhance job skills and knowledge (Magnet Force #'s 4,14).,10% Discharge Planning - Reviews and evaluates discharge plans with the multidisciplinary teams, the patient, and family to promote timely and appropriate discharge(Magnet Force #'s 5,10,11,13).,20% Plan of Care - Obtains information from patient and/or family to develop an appropriate patient educational plan. Formulates, implements and continually revises patient plan of care based on ongoing assessment of the patient, information acquired from patient's database, and resources within the medical center (Magnet Force #'s 2,6). Collaborates with other members of the health care team to promote effective patient care (Magnet Force #13).,20% Communication - Performs required documentation and communicates in a competent and effective manner to meet patient and/or family needs and regulatory requirements. Maintains strict confidentiality of patient, employee and department information to ensure patient rights (Magnet Force #'s 3,8,13).,40% Patient Care & Assessment - Clarifies and implements physician, physician assistant and/or advance practice nurse orders by administering medications and performing treatments and testing as prescribed by the physician, and documents accordingly (Magnet Force #12). Recognizes symptoms and establishes priorities consistent with patient's condition (Magnet Force #9). Documents ongoing assessments of patient's physical and emotional status in order to address patient needs appropriately (Magnet Force #'s 5,6). Demonstrates mastery of required unit-based competencies and orientation goals.
