This position is part of the hospital implementation team and acts as a temporary Dialysis Coordinator (DC) during a new hospital start, provides coverage to hospital partners in a dialysis coordinators' absence, and provides regular operating reviews on local market performance. This position contributes to strategy development and implementation to improve the AKI/ESKD patient discharge process as well as builds relationships with the kidney care community. This role supports the initiation of Patient Pathways programs at new hospital partner locations. This role will partner with the Operations Manager and hospital partners to identify best practices and work with Quality Assurance to spread those practices to other teams. A successful candidate will be engaged in the teammate and patient education processes, quality assurance, and growth.

This position requires up to 100% travel within the local market as well as across the U.S. Current driver's license (including RealID), reliable transportation, and vehicle insurance in the state of residence required

Experience:

  • 2-4 years of experience is required; including at least 1 year of discharge planning and/or case management support experience.
  • High School diploma required; Bachelor's degree in health care related field is preferred.
  • Case Management Certification, LCSWA, or Certified Nephrology Nurse - preferred.
  • Minimum of two (3) years' experience demonstrating a strong understanding of commercial insurance plans and benefits (PPO, HMO, POS, EPO, Indemnity) as well as government insurance programs (Medicare, Medicaid) required
  • Ability to routinely and successfully pass background checks and drug screens, including nicotine

Physical requirements can vary. These must be reviewed with management. However, in general, the position requires the following activities:

  • Ability to lift a minimum of 5 lbs to a max of 50 lbs unassisted and able to stand, sit, stoop, walk, stretch, reach, and use full range of body motions.
  • Manual dexterity as required for specific equipment and ability to use computer keyboard and peripherals for an extended period of time.
  • Ability to speak/hear for presentation and frequent telephone conversations.
