REQUIREMENTS

• Master's Degree in a Human Services field such as social work, psychology, rehabilitation, activity therapies, counseling or education and 1 year of paid professional behavioral health experience; OR a Bachelor's Degree with major course work in sociology, social work, psychology, gerontology, science, history, criminal justice, theology, nursing, counseling or education and 3 years of paid behavioral health experience. Other fields of study may be considered with coursework of at least 12 credits in Human Services.

Act 33/34 and FBI clearances.

Drug Screening

Valid driver's license and access to transportation and the ability to drive a company vehicle.

Schedule - This is an exempt position

Monday to Friday 9am to 5pm

Weekends off

ABOUT THE ROLE

We're looking for a Service Coordinator to serve on the front lines of one of our most integral programs, helping adults with serious and persistent mental illnesses/emotional disorders get access to services, build support networks, and resolve conflicts.

As a service coordinator you will engage in the following activities:

• Advocate for and with the individual being served to ensure responsiveness from natural, community, geriatric and specialized services/supports. Advocacy includes providing information, removing barriers, creating options and resolving problems.

• Maintain an accurate and timely record of Service Coordination activity by completing service documents within 24 hours. Reviewing charts for compliance with regulations and preparing for formal audits

• Ensures individuals being served needs are met through the utilization of natural supports (family, friends), community and geriatric services and specialized services (MH/MR, supported employment, OVR, D&A). Assists individual and family to identify, link, access and coordinate such resources.

• Provide on-site support for all in-patient MH commitments by ensuring all alternatives to hospitalization have been considered first then advocating for admission.

• Provides culturally competent services with consideration for the individual's racial, religious, sex, sexual orientation, age and ethnic background and identification.

• Attends training programs provided through the State, County and Agency to assure that the incumbent is up to date on new approaches, best practices and recovery oriented services. 37.5 annual training is required.

• Ensures the frequency of contact with the individual being served is based on the individual's needs, but at the very least once a month. Provide a full range of Service Coordination services to a designated caseload maximum of 25 individuals for adults.
