This role is responsible for providing company-wide management and oversight to social determinants of health (SDOH) including planning, development, implementation, and reporting of SDOH Coordinated Care Organization (CCO) requirements. The role requires cross-functional collaboration and management of interdependencies between multiple departments and related work streams. This role will: 1) Ensure community health strategies and activities related to advancing SDOH work in all CCO regions are coordinated, measurable, and effective; 2) Utilize REALD, SOGI, and other applicable data points to identify opportunities to advance SDOH strategies; 3) Ensure the reporting provided to regulators is of the highest possible quality, submitted timely, and well-coordinated with other related work; 4) Coordinate regional engagement efforts related to community information exchange for social needs screening and referrals.
Essential Responsibilities:
- Support SDOH community capacity-building in all regions.
- Assist VP of Community Health, Social Needs, and Health Equity, Director of Community Health Strategy, and other business owners to develop and achieve strategies to meet the Oregon Health Authority (OHA) SDOH requirements.
- Assist VP of Community Health, Social Needs, and Health Equity and Director of Community Health Strategy to work with internal departments and external community partners to implement SDOH engagement strategies and processes.
- Utilize REALD, SOGI, and other data points to identify opportunities to advance SDOH, health equity, and Health Related Social Needs (HRSN) strategies.
- Work closely with the Director of Community Health Strategy, Health Equity Strategist, HRSN Program Manager, and the Medicaid Administration team to align efforts, internally and externally, and on CCO contract deliverables.
- Support and coordinate with Health Equity Strategist in the development, implementation, and reporting lifecycle of Transformation and Quality Strategy (TQS) and Health Equity Plan (HEP) deliverables.
- Support HRSN Program Manager in developing organizational competencies to provide climate, housing, and food and nutrition supports, including providing technical assistance to HRSN Service Providers in all regions.
- Coordinate regional engagement efforts related to community information exchange, including support for HRSN and SDOH and Social-Emotional Quality Incentive Measures (QIM).
- Assist VP of Community Health, Social Needs, and Health Equity and Director of Community Health Strategy with coordination of SDOH Leadership Team meetings.
- Facilitate presentations and workgroups with internal and external partners to advance SDOH, HEP, and TQS strategies and projects.
- Collaborate and coordinate relevant SDOH initiatives between lines of business (Medicaid, Medicare, D-SNP, commercial).
- Adhere to PacificSource project management approaches, project lifecycle methodologies, and timelines.
Supporting Responsibilities:
- Meet department and company performance and attendance expectations.
- Participate in and support project teams led by other departments and provide necessary input to support the goals of colleagues.
- Follow the PacificSource privacy policy and HIPAA laws and regulations concerning confidentiality and security of protected health information.
- Perform other duties as assigned.
SUCCESS PROFILEWork Experience: Minimum of 4 years of experience and/or training in healthcare, public health, or community-based organizations, preferably related to social needs, such as housing and/or food insecurity. Experience working to advance social determinants of health, diversity, equity, and inclusion efforts in healthcare or community systems. Experience preferred in community networking and/or facilitating groups. Data analysis experience required.
Education, Certificates, Licenses: Bachelor's degree required. Master's degree in public health or related field preferred. Cultural responsivity training experience preferred.
Knowledge: Applicants who have lived experience as a member of an underserved/underrepresented group of people are strongly encouraged to apply. Knowledge and understanding of social determinants of health and fundamentals of diversity, equity, inclusion, and belonging. Ability to work both independently and collaboratively. Project management and implementation to reach desired outcomes. Skilled in relationship building and community collaboration. Knowledge in presentation development and writing technical reports preferred. Organizational skills with solid experience in using computers and various software applications. Demonstrated successful communication skills, including public presentation, training, meeting facilitation, and interpersonal skills. Maintain high level of knowledge of company products, health reform trends at the Federal and State levels, and the insurance industry. Demonstrated skills with the following software: Microsoft Word, PowerPoint, Excel, and Visio. Strong analytical and problem-solving skills. Ability to work independently with minimal supervision. Bilingual in Spanish preferred.

Environment: Work inside in a general office setting with ergonomically configured equipment. Travel is required approximately 50% of the time. Rare overnight travel.
