Mearkey is seeking a Social Worker/Behavioral Health Navigator to join our Behavioral Health team, in Delaware County, PA.

POSITION OVERVIEW

Provide behavioral health assessment of the psychological and social factors affecting an individual and promote a person's awareness of how these factors affect overall wellness including substance use issues.

Encourage a person to identify his/her physical and behavioral health care concerns.

Educate medical providers about behavioral healthcare needs.

Assess the level of behavioral health care needs and provide coordination when identified.

Perform group, family and educational intervention as needed.

Meet with people in recovery (PIRs) on demand and via scheduled appointments to assist with linkage to external resources such as, but not limited to: housing, recovery based supports such as 12-step meetings, medical benefits, PCP, specialty doctors, food banks.

Complete Logisiticare forms for PIRs needing bus passes, gas reimbursement, and transportation assistance.

Effective and timely communication with Supervisors and Counselors on PIRs needs and as resources or obtained and/or changed.

Assist Counselors as needed with referrals to a higher level of care and with transfers limited to ensuring ROIs are obtained and paperwork sent timely.

Ability to educate and encourage resistant PIRs to participate in therapy services.

Other job duties as required.

Master's degree in Social Work, Psychology, Nursing, Counseling or related clinical field of study.

Minimum 3 years direct mental health care experience.

Training and professional experience in co-occurring mental health and substance use disorders.

Experience in Motivational Interviewing, co-occurring disorders, trauma informed care or other related areas preferred but not required.

Experience in integrated primary and behavioral health care a plus.

Training in integrated primary and behavioral health care is part of the initial employment orientation.
