Description:

Provides psychosocial clinical intervention, consultation and education to patients/families to assure comprehensive services and continuity of care of patients. Contributes to the training of healthcare professionals to enhance the educational programs of the Loyola University Health Care System.

Works collaboratively with the team for coordinated care across the continuum. Maintains, and utilizes clinical resources and/or programs that address the psychosocial and medical needs of the patient, making referrals as necessary. Documents interventions and communicates to the patient's healthcare team in a timely fashion. Attending and contributing to the assigned patient care conferences to ensure comprehensive care plan.

Position Requirements:

Minimum Education

  • Required: Masters Degree
  • Specify Degree(s): Masters of Social Work
  • Residency: Internship in Inpatient Hospital Setting Preferred Minimum Experience:

Minimum Education
  • Required: None
  • Preferred: 1-2 years of previous job-related experience
Detail s: Expected to have CMA certification within 3 years of hire

Licensure/Certifications:
  • Required:Licensed Social Worker
  • Preferred:Licensed Clinical Social Worker
  • Other: Expected to have CMA certification within 3 years of hire.
