The Brooklyn ACT program provides an integrated set of evidenced based treatment, rehabilitation, case-management and support services to individuals residing in Shelters and/or living in supportive housing programs or their own residences in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care.

POSITION OVERVIEW:

The Team Director takes lead as a member of the ACT Team, which promotes a team based approach in its work with adults with behavioral health disorders living in homeless shelters or in the community. S/he is responsible for ensuring clinical services are provided using evidence based approaches and best practice, and managing day to day operations to ensure the program runs smoothly and with minimal disruptions to client services. The Team Director provides on-going training, supervision, and guidance to the clinical and office staff.
  • Team Director dedicates 50% of his/her time to direct patient services in the community and dedicates the remaining 50% to administrative and supervisory duties.
  • Responsible for the day to day clinical and operational operations of the ACT team and ensures adequate coverage to meet the clinical needs of the clients and to be in compliance with the contract.
  • Helps Program Director with hiring and orienting new staff; conducting performance evaluations, providing training through regular supervision and team meetings; and addressing problematic behaviors and performance issues.
  • Responsible for insuring all reporting requirements are being adhered to in a timely manner. This includes staff documentation, reporting into various systems that interact with ACT as well as conducting internal reviews of documentation.
  • Is a key staff resource for community outreach efforts and may perform such outreach and establish linkages with shelters and other groups, individuals or organizations which may be helpful to our clients.
  • Provide after-hours emergency coverage via cell phone on a rotating basis.

EDUCATIONAL

A Master's Degree or higher in social work, psychology, rehabilitation counseling or a related field plus NY State licensure as
  • Licensed Master of Social Work (LMSW), Licensed Clinical Social Work (LCSW), Licensed Mental Health Counseling (LMHC), Licensed Marriage and Family Therapy (LMFT), Licensed Psychologist or Licensed Creative Arts Therapist.
